Rosuvastatin (Crestor®) is a lipid-lowering drug that is used as a statin or as a prohormone for patients with heterozygous familial hypercholesterolemia (HHD). It also is used in HHD patients with hypercholesterolemia. In addition to its primary use as a statin, Crestor has also been approved by the United States Food and Drug Administration (FDA) for the treatment of patients with familial hypercholesterolemia and the treatment of HHD [
]. This study aimed to compare the efficacy of rosuvastatin (Crestor®) with rosuvastatin and its alternatives in patients with HHD, focusing on the effectiveness of statins in patients with HHD. This retrospective cohort study included patients who received rosuvastatin, either alone or in combination with other lipid-lowering drugs, from 1995 to 2015. The study assessed the effectiveness of rosuvastatin, rosuvastatin plus apixaban, or combination therapy and the effectiveness of combination therapy on HHD in patients with HHD. The primary end-points were the change in HHD-assessed serum lipid levels, defined as total cholesterol and LDL-cholesterol, and waist circumference, and the change in HHD-assessed serum triglycerides. Secondary end-points included change in the primary endpoint, change in HHD-assessed serum lipids, and changes in HHD-assessed serum triglycerides.
A total of 5,724 patients were identified from the medical records of patients with HHD and HHD-associated cardiovascular disease (CVD). The median age of the study population was 60 years (95% CI, 59–74 years). The patients had a mean LDL-cholesterol of ≥ 130 mg/dL (95% CI, ≥ 120–130 mg/dL) and a mean HDL-cholesterol of ≥ 35.9 mg/dL (95% CI, 34.4–33.6 mg/dL). The patients were divided into 3 groups according to the duration of statin therapy, starting rosuvastatin or beginning combination therapy. The duration of statin therapy was 8.5 years (95% CI, 6.5–11.1 years), and the duration of combination therapy was 3.9 years (95% CI, 2.7–5.3 years). Among the 5,724 patients with HHD and HHD-associated CVD, the mean LDL-cholesterol and mean HDL-cholesterol levels were significantly higher than those of the control group (
). The mean duration of combination therapy was 3.9 years (95% CI, 2.7–5.3 years), and the mean duration of statin therapy was 3.9 years (95% CI, 2.7–5.3 years). The patients who used combination therapy had more side effects than those who used statin alone (
In a retrospective study, rosuvastatin plus apixaban, or combination therapy, was superior to rosuvastatin alone in the reduction of lipid levels in patients with HHD. The study demonstrated that rosuvastatin was able to reduce the lipid levels and improve the lipid-lowering effects of statins, including cholesterol levels.
Rosuvastatin is an oral lipoprotein-lowering agent. In patients with HHD, the rate of lipid accumulation increases with the duration of treatment. Although rosuvastatin is effective as a statin in patients with HHD, its use in HHD patients has been linked to increased levels of lipid-lowering drugs, including cholesterol and triglycerides, which are the main circulating lipid components. Therefore, rosuvastatin is considered a potential therapeutic option for patients with HHD [
In patients with HHD, rosuvastatin was found to be superior to apixaban in reducing serum cholesterol levels and improving the lipid-lowering effects of statins [
In addition, rosuvastatin is also effective as an adjunct to other lipid-lowering drugs in HHD patients. The efficacy of rosuvastatin as an adjunct to other lipid-lowering agents has been investigated in patients with HHD [
This study demonstrated that rosuvastatin can be effective in HHD patients who were treated with either rosuvastatin alone or with other lipid-lowering agents.
